insurance and selected Preferred Provider

cosmetic dentistry covered by insurance? |

Generally not, but some procedures
may be partially covered if decayed or fractured
teeth are involved in the cosmetic restoration
of your smile. |

Flexible Spending Account (FSA) for dental
procedures? |

Yes, We can provide you with an estimate
of the fee for your smile makeover so that
you can arrange to deposit the necessary
funds into you FSA. |

How can I pay
for my dental services? |
| A: |
We accept cash, check or credit card for
your insurance co-pay due at the time of
service. Procedures over $300 may be financed
with us through Dental Fee Plan. This plan
offers qualified patients 12 or 18 months
interest free financing, or you may make
extended payments for up to 60 months for
a very nominal interest rate. We also offer
a 10% reduction in fee for cases over $300
that are paid in cash at the time of service
or an 8% reduction in fee with credit card
payments. |

Do you do dental work
on children? |

Yes. In general the dental procedures
are not difficult, although in some cases
patient management can be an issue in young
children. In those cases, we will refer
your child to a Pedodontist (children’s
specialist). |

In general, how many
visits does a cosmetic makeover require? |
| A: |
Typically you can have your smile makeover
in just two appointments. You will be amazed
immediately (first visit) since we will
provide you with cosmetic temporaries shaped
like your final veneers (second visit). |

This is my first visit
with you, how should I get my past dental
records transferred to your office? |

Due to Federal Privacy laws (HIPPA), you
must have your own records transferred by
signing a release for with your previous
dentist. Unfortunately we are no longer
able to request records on your behalf. |

In the case of a dental
emergency at night or on the weekends, who
should I call? |

The doctors are always available to speak
with you concerning emergencies. Just call
the office, and you will find all personal
contact information on our answering service. |
